Information we collect
We collect two types of information:
Information you provide
When you contact us, request an appointment, or apply for a training position, you provide information including your name, email address, phone number, and any other details you choose to share. You may also upload documents (e.g., a CV) when applying for a training position.
Information collected automatically
When you visit our website, we automatically collect certain technical information including your IP address, browser type, device type, pages visited, time spent, referring URL, and other analytics data. We use cookies and similar technologies — see our Cookie Policy for details.
How we use your information
We use the information we collect to:
- Respond to inquiries, schedule appointments, and provide care;
- Verify insurance benefits and process payments;
- Communicate with you about appointments, billing, and our services;
- Evaluate training applications;
- Improve our website, content, and services;
- Comply with legal obligations and protect our rights.
We do not sell your personal information to third parties. We do not engage in cross-context behavioral advertising.
How we share information
We share information only as necessary to provide our services or as required by law. Personnel of Pasadena Clinical Group — including employees, clinicians, supervisors, supervisees, trainees, interns, volunteers, agents, contractors, and business associates engaged in our work — may access information to the extent necessary to perform their roles, and are contractually and policy-bound to maintain the confidentiality of that information consistent with HIPAA, the California Medical Information Act (CMIA), the California Consumer Privacy Act / California Privacy Rights Act (CCPA/CPRA), and our internal policies. Common situations include:
- With our workforce members (employees, clinicians, supervisors, supervisees, trainees, interns) on a need-to-know basis to provide care, scheduling, billing, and administrative support;
- With service providers and business associates (e.g., our website host, form processing infrastructure, analytics provider, secure email and electronic-health-record vendors, payment processors, telehealth platform, billing and insurance-clearinghouse vendors) who are contractually bound to protect your information under written agreements that meet HIPAA, CMIA, and CCPA/CPRA requirements;
- With your insurance plan, when you provide insurance information for benefits verification, prior authorization, or claims;
- When required by law or in response to valid legal process (subpoenas, court orders);
- To protect the rights, safety, or property of any Released Party (including our employees and contractors), our clients, or others; and
- In connection with a merger, acquisition, reorganization, financing, sale of assets, or similar transaction, subject to applicable confidentiality obligations.
Sharing of your Protected Health Information (PHI) is governed by our HIPAA Notice of Privacy Practices, which incorporates HIPAA, the HITECH Act, and the California Medical Information Act (CMIA, Civil Code §56 et seq.).
We do not sell your personal information for monetary consideration, and we do not share your personal information for cross-context behavioral advertising. If we ever change these practices, we will update this Privacy Policy and provide a "Do Not Sell or Share My Personal Information" mechanism in compliance with CCPA/CPRA.
Forms and bot protection
Our online forms use Cloudflare Turnstile for bot protection. Turnstile collects limited technical information from your browser to verify that you are not an automated bot. This information is used solely for security purposes. See Cloudflare's privacy policy for details.
Your California privacy rights (CCPA / CPRA)
If you are a California resident, you have the following rights under the California Consumer Privacy Act (CCPA), as amended by the California Privacy Rights Act (CPRA):
- Right to know: The right to know what personal information we collect, use, disclose, and (if applicable) sell or share.
- Right to delete: The right to request deletion of personal information we have collected, subject to certain exceptions.
- Right to correct: The right to request correction of inaccurate personal information.
- Right to opt-out of sale or sharing: We do not sell or share personal information for cross-context behavioral advertising. There is no opt-out to exercise.
- Right to limit use of sensitive personal information: The right to limit our use of sensitive personal information to that which is necessary to provide the services you requested.
- Right to non-discrimination: We will not discriminate against you for exercising any of these rights.

You may designate an authorized agent to make a request on your behalf. The agent must provide written authorization, and we may require you to verify your own identity directly.
Data retention
We retain personal information only as long as necessary to provide our services, comply with legal obligations, resolve disputes, and enforce our agreements. Clinical records (PHI) are retained according to California state law and our retention schedule, which may extend beyond seven years depending on the type of record.
Security
We use reasonable administrative, technical, and physical safeguards to protect personal information. However, no method of transmission over the internet or electronic storage is 100% secure. We cannot guarantee absolute security.
Children's privacy
Our website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 in violation of the Children's Online Privacy Protection Act (COPPA). California law also affords heightened protections to consumers under 16. If you believe a child under 13 has provided personal information to us, please contact us so we can delete it.
Liability and dispute resolution
Your use of this website and any disputes arising out of or relating to the collection, use, or sharing of information are subject to the limitation-of-liability, indemnification, mediation, binding individual arbitration, class-action waiver, jury-trial waiver, governing-law, and venue provisions in our Terms & Conditions. Those provisions are incorporated into this Privacy Policy by reference and extend to all Released Parties — including, without limitation, all employees, clinicians, contractors, agents, and business associates of Pasadena Clinical Group. Nothing in this Privacy Policy limits any rights you may have under HIPAA, CMIA, or CCPA/CPRA that, by law, may not be limited.
Changes to this policy
We may update this Privacy Policy from time to time. The current version will always be posted on this page with a "Last updated" date. We encourage you to review the policy periodically.
